Return Eligibility and General Policy
At Surgyx, we are committed to ensuring your satisfaction with every purchase. All our surgical instruments are guaranteed to be shipped without defect. However, if you receive a product that is damaged, incorrect, or if you are not satisfied for any reason, we offer a clear and fair return policy.
All return requests must be authorized by a Surgyx sales or customer service representative. Unauthorized returns will not be accepted.
How to Request a Return
To initiate a return, you must first apply for a Return Merchandise Authorization (RMA) number. This can be done by filling out the request form at:
Our returns department will review your request and provide you with a downloadable RMA form, which must be included in your return package. The RMA authorization will remain valid for 30 days from the date of issue.
Please Note: Any packages received without an RMA number written on both the shipping label and original invoice will be refused.
Required Information for All Return Requests
When applying for a return, please have the following details ready:
Purchaser’s full name, phone number, and address
Invoice number and date
Purchase order number (if applicable)
Catalog number and quantity of items
Clear reason for the return
Return Window and Conditions
You may return your product within 30 days of delivery for a refund.
Used instruments cannot be returned.
Items returned after 30 days will be subject to a minimum 25% restocking fee.
Special order or customized products are non-returnable and non-refundable.
Returned items must be in their original condition and packaging, including all documentation and tags. Returns not meeting these requirements may incur an additional restocking fee or be rejected.
Shipping Instructions for Returns
Return shipping is the responsibility of the customer. We recommend using a secure, trackable courier service such as UPS, FedEx, DHL, EMS, or Kerry Express. Surgyx is not responsible for any packages lost in transit.
Include the issued RMA number on the shipping label and inside the package
Ship the product in its original packaging with all accessories and documents
Ensure the product is properly insured and packed to avoid damage in transit
Refund Process
Once we receive and inspect your return, we will notify you of the status. Please allow 3 to 4 weeks for the return to be processed and for your refund to be issued.
Refunds will only be issued if the product is received in acceptable, unused condition. No credits will be given for instruments that are damaged, used, or returned without proper documentation.
